Why ectopic pregnancy can hurt at the shoulder

An ectopic pregnancy develops outside the uterus, most often in a fallopian tube. If bleeding occurs inside the abdomen, blood can irritate the diaphragm beneath the lungs. The nerve signal may be felt at the shoulder tip where the shoulder ends and the arm begins. This is referred pain.

The right side does not settle the question

The side of shoulder pain cannot locate an ectopic pregnancy. The pain may be felt at the right shoulder, left shoulder, or both because it is referred from diaphragm irritation. One-sided lower abdominal pain can occur with ectopic pregnancy, but shoulder side does not reliably match the affected fallopian tube.

Symptoms alone cannot diagnose the pregnancy location. Clinicians may use an examination, blood tests for human chorionic gonadotropin, commonly called hCG, and ultrasound. Follow-up may be needed when an early scan does not yet show where the pregnancy is developing.

Other causes are considered after urgent causes
